The forms below are designed to inform users of the proper use of technology at Hopewell-Loudon Schools.  They are required in order for students to use computer or server services from the District.

This form is required and informs the student, parent of the do’s and don'ts of using electronic devises at Hopewell-Loudon Schools.  This should be read to the student by the parent. 

This form is required for all staff members and advises them of the acceptable use of electronic devises at Hopewell-Loudon Schools.

These forms are required for teachers who wish to purchase a computer under Ohio’s tax exempt laws.

This authorization form gives permission to the school by the parent to use student photos, video for production purposes.  Such as channel 14, DVD’s, yearbook or local newspapers.

This form is required and informs staff members, students and parents of proper and improper use of the schools email.
